Uglednog hrvatskog imunologa, prof. dr. sc. Zlatka Trobonjaču s Katedre za fiziologiju, imunologiju i patofiziologiju Medicinskog fakulteta u Rijeci, pitali smo što misli o dodatnom cijepljenju, koje nas vjerojatno čeka na jesen?
Ima li se smisla već sada docijepiti četvrtom dozom ili čekati nova cjepiva prilagođena novim varijantama?
– U pripremi su tzv. bivalentne vakcine, temeljene na mRNK tehnologiji, koje će pokrivati originalni soj SARS-CoV-2 virusa i omikron virus.
Omikron se mutacijama razgranao, pa od onih početnih virusnih varijanti BA.1 i BA.2 danas epidemiju u većini zemalja, kao i u našoj zemlji, vodi nešto drukčija BA.5 varijanta.
U bivalentnom cjepivu nalazimo mRNK BA.1 varijante, no ispitivanja su pokazala da imunost koja se razvije cijepljenjem pokriva i sada aktualnu BA.5 varijantu.
Ima li smisla četvrta doza?
Na pitanje ima li smisla već sada uzeti četvrtu dozu ili čekati ovo bivalentno, omikronu prilagođeno cjepivo, teško je uniformno odgovoriti, jer odgovor ovisi ne samo o cijepnom statusu osobe ili statusu preboljenja, nego i starosti te eventualnom postojanju kroničnih bolesti, koje pogoduju razvoju teških oblika COVID-a.
Drugim riječima, naši stariji sugrađani, koji boluju od kroničnih kardiovaskularnih bolesti, metaboličkih bolesti, poput dijabetes melitusa, malignih bolesti, teških autoimunosnih bolesti, oni koji se nalaze na imunosupresivnoj ili kemoterapiji, a kojima je prošlo više od pet mjeseci od treće doze ili preboljenja omikron virusa, mogu primiti četvrtu dozu ne čekajući dolazak prilagođenog cjepiva.
Mlađe i zdravije osobe mogu čekati, a hoće li se uopće trebati docijepiti, ovisit će o obilježjima epidemije na jesen.
Na jesen stižu novi lijekovi
Već su nam na raspolaganju neki lijekovi za liječenje COVID-a, ali stižu i neki novi. Koliko su oni učinkoviti i ima li nade da se konačno obračunamo s ovim groznim virusom?
– Pristupi u liječenju COVID-a ovise o više faktora, poput faze bolesti, starosti bolesnika, vrsti podležećih bolesti, eventualnoj terapiji itd. Mnogi od tih faktora služe liječnicima u procjeni mogućnosti razvoja teške COVID bolesti i neželjenih ishoda.
Od antivirusnih lijekova koji se koriste ili su se koristili, valja spomenuti monoklonska protutijela casirivimab i imdevimab, kao i sotrovimab, koji su, nažalost, izgubili učinkovitost pojavom novih imunoevazivnih sojeva omikron virusa.

Tako BA.2 varijanta, kao i BA.4 i BA.5 varijante, višestruko slabije reagiraju na terapiju ovim monoklonskim protutijelima u odnosu na prethodne epidemijske sojeve, poput alfa, delta, te omikron BA.1 virusa.
Visoka učinkovitost Paxlovida
Noviji lijek koji je pokazao visoku učinkovitost u prevenciji razvoja teške bolesti je Paxlovid (kombinacija nirmatrelvira i ritonavira). Nirmatrelvir je inhibitor virusne glavne protease, enzima koji virus koristi tijekom svoje replikacije, dok mu ritonavir pojačava i produljuje djelovanje.
Smanjuje učestalost hospitalizacija i smrti za gotovo 90 posto, a ima ga smisla primjenjivati tijekom prvih pet dana od početka bolesti upravo kod onih ljudi za koje liječnici mogu pretpostaviti da bi mogli razviti tešku bolest.
Drugi lijek koji se recentno pojavio je molnupiravir s učinkovitošću u smanjenju učestalosti hospitalizacija i smrti za oko 30 posto. Molnupiravir je nukleozid koji se ugrađuje u virusni genom tijekom replikacije izazivajući niz mutacija koje virus ne može preživjeti.
Mogu pomoći, ali nisu garancija
Zbog takvog mutagenog potencijala ne smije se davati trudnicama jer bi mogao izazvati oštećenje ploda. Od tih novijih, i vrlo skupih lijekova, mogu spomenuti i lijekove koji se koriste kod teških oblika COVID-a, tzv. citokinskim olujama, napose za blokadu citokina interleukina-6, poput tocilizumaba ili sarilumaba.
Ovi lijekovi mogu pomoći, ali nisu garant da će doći do izlječenja. Ako krenemo od temeljne medicinske premise, bolje spriječiti nego liječiti, to bi u slučaju COVID-a 19 značilo: bolje se cijepiti nego se liječiti u bolnici superskupim lijekovima ograničene učinkovitosti.
Svaku bolest, pa i ovaj nesretni COVID-19 treba prvenstveno sagledavati iz pozicije bolesnika, što nas vodi prema cijepljenju kao najučinkovitijoj zaštiti.

Uglednog hrvatskog imunologa, prof. dr. sc. Zlatka Trobonjaču s Katedre za fiziologiju, imunologiju i patofiziologiju Medicinskog fakulteta u Rijeci, pitali smo što misli o dodatnom cijepljenju, koje nas vjerojatno čeka na jesen?
Ima li se smisla već sada docijepiti četvrtom dozom ili čekati nova cjepiva prilagođena novim varijantama?
– U pripremi su tzv. bivalentne vakcine, temeljene na mRNK tehnologiji, koje će pokrivati originalni soj SARS-CoV-2 virusa i omikron virus.
Omikron se mutacijama razgranao, pa od onih početnih virusnih varijanti BA.1 i BA.2 danas epidemiju u većini zemalja, kao i u našoj zemlji, vodi nešto drukčija BA.5 varijanta.
U bivalentnom cjepivu nalazimo mRNK BA.1 varijante, no ispitivanja su pokazala da imunost koja se razvije cijepljenjem pokriva i sada aktualnu BA.5 varijantu.
Ima li smisla četvrta doza?
Na pitanje ima li smisla već sada uzeti četvrtu dozu ili čekati ovo bivalentno, omikronu prilagođeno cjepivo, teško je uniformno odgovoriti, jer odgovor ovisi ne samo o cijepnom statusu osobe ili statusu preboljenja, nego i starosti te eventualnom postojanju kroničnih bolesti, koje pogoduju razvoju teških oblika COVID-a.
Drugim riječima, naši stariji sugrađani, koji boluju od kroničnih kardiovaskularnih bolesti, metaboličkih bolesti, poput dijabetes melitusa, malignih bolesti, teških autoimunosnih bolesti, oni koji se nalaze na imunosupresivnoj ili kemoterapiji, a kojima je prošlo više od pet mjeseci od treće doze ili preboljenja omikron virusa, mogu primiti četvrtu dozu ne čekajući dolazak prilagođenog cjepiva.
Mlađe i zdravije osobe mogu čekati, a hoće li se uopće trebati docijepiti, ovisit će o obilježjima epidemije na jesen.
Na jesen stižu novi lijekovi
Već su nam na raspolaganju neki lijekovi za liječenje COVID-a, ali stižu i neki novi. Koliko su oni učinkoviti i ima li nade da se konačno obračunamo s ovim groznim virusom?
– Pristupi u liječenju COVID-a ovise o više faktora, poput faze bolesti, starosti bolesnika, vrsti podležećih bolesti, eventualnoj terapiji itd. Mnogi od tih faktora služe liječnicima u procjeni mogućnosti razvoja teške COVID bolesti i neželjenih ishoda.
Od antivirusnih lijekova koji se koriste ili su se koristili, valja spomenuti monoklonska protutijela casirivimab i imdevimab, kao i sotrovimab, koji su, nažalost, izgubili učinkovitost pojavom novih imunoevazivnih sojeva omikron virusa.

Tako BA.2 varijanta, kao i BA.4 i BA.5 varijante, višestruko slabije reagiraju na terapiju ovim monoklonskim protutijelima u odnosu na prethodne epidemijske sojeve, poput alfa, delta, te omikron BA.1 virusa.
Visoka učinkovitost Paxlovida
Noviji lijek koji je pokazao visoku učinkovitost u prevenciji razvoja teške bolesti je Paxlovid (kombinacija nirmatrelvira i ritonavira). Nirmatrelvir je inhibitor virusne glavne protease, enzima koji virus koristi tijekom svoje replikacije, dok mu ritonavir pojačava i produljuje djelovanje.
Smanjuje učestalost hospitalizacija i smrti za gotovo 90 posto, a ima ga smisla primjenjivati tijekom prvih pet dana od početka bolesti upravo kod onih ljudi za koje liječnici mogu pretpostaviti da bi mogli razviti tešku bolest.
Drugi lijek koji se recentno pojavio je molnupiravir s učinkovitošću u smanjenju učestalosti hospitalizacija i smrti za oko 30 posto. Molnupiravir je nukleozid koji se ugrađuje u virusni genom tijekom replikacije izazivajući niz mutacija koje virus ne može preživjeti.
Mogu pomoći, ali nisu garancija
Zbog takvog mutagenog potencijala ne smije se davati trudnicama jer bi mogao izazvati oštećenje ploda. Od tih novijih, i vrlo skupih lijekova, mogu spomenuti i lijekove koji se koriste kod teških oblika COVID-a, tzv. citokinskim olujama, napose za blokadu citokina interleukina-6, poput tocilizumaba ili sarilumaba.
Ovi lijekovi mogu pomoći, ali nisu garant da će doći do izlječenja. Ako krenemo od temeljne medicinske premise, bolje spriječiti nego liječiti, to bi u slučaju COVID-a 19 značilo: bolje se cijepiti nego se liječiti u bolnici superskupim lijekovima ograničene učinkovitosti.
Svaku bolest, pa i ovaj nesretni COVID-19 treba prvenstveno sagledavati iz pozicije bolesnika, što nas vodi prema cijepljenju kao najučinkovitijoj zaštiti.
