     Zbog svoje brze prenosivosti, najveća briga epidemiologa dosad je bila delta varijanta koronavirusa, još uvijek najdominantnija mutacija SARS-CoV-2 virusa u svijetu. Fokus znanstvenika se, međutim, snažno pomiče na delta-plus soj koji je jučer potvrđen i u Hrvatskoj. Soj je registriran u Čakovcu i nije baš veliko iznenađenje što se konačno pojavio i kod nas, pošto je u Europu stigao još prije ljeta.

    Riječ je o varijanti AY.4.2 koja je dosad otkrivena u više od 40 zemalja: od Danske, Velike Britanije, Austrije, Španjolske, pa preko niza istočnoeuropskih zemalja i Rusije do SAD-a, Izraela i Indije. Mutacije u genomu proteina šiljka daju mu veću  imunoevazivnost, odnosno mogućnost izbjegavanja čovjekova imunosnog odgovora. U kojoj mjeri je sposobniji od ranijih sojeva zaobići obrambene sustave organizma još uvijek nije dovoljno istraženo. Prema dosad provedenim kliničkim istraživanjima, delta plus djeluje nešto razornije na pluća i izaziva jače poteškoće u cijelom respiratornom traktu, te inicira probavne smetnje učestalije od ostalih sojeva.

     "Povećanje prijenosa AY.4.2 prati se od srpnja. U tijeku su epidemiološke i laboratorijske studije kako bi se procijenilo je li došlo do promjene karakteristika prijenosa ili smanjenja učinka antitijela na odbacivanje virusa“, objavili su nedavno iz Svjetske zdravstvene organizacije, dodajući da je najveći postotak zaraženih ovim virusom iz Ujedinjenog Kraljevstva. Britanska agencija za zdravstvenu sigurnost UKHSA prošlog je tjedna označila AY.4.2 kao "soj pod istragom", ali unatoč njegovom snažnom širenju još ipak nije službeno proglašen „zabrinjavajućim sojem“.

     Stručnjaci nisu posve sigurni što je dovelo do jačeg širenja ovoga soja u Britaniji i je li to posljedica mutacije ili, možda, tamošnjih minimalnih epidemioloških mjera. Trenutne britanske mjere protiv covida su vrlo slabe, praktički ih nitko i ne nadzire, pa se i ne zna u kojoj mjeri je to razlog jačem širenju ove inačice.

     Poznato je kako najrasprostranjenija delta varijanta korone dosad ima preko 50 svojih podvarijanti. Sve one imaju tipične mutacije za delta virus, ali postoje i dodatne mutacije koje ih onda svrstavaju u njegove podvarijante. Takav soj je i AY.4.2 jer ima dvije dodatne mutacije u proteinu šiljka - L452R i P871R - uz čiju se pomoć brže širi. Britansko iskustvo od početka srpnja, kada je otkriven, govori da od svih izolata obuhvaća oko 10 posto i bilježi trend porasta. Na Otoku je najviše slučajeva delte plus zabilježeno kod djece, no to medicinare puno ne iznenađuje jer je riječ o populaciji koja je najslabije procijepljena. 

     Ranije mutacije korone izazivale su u pravilu skokovite valove u broju novozaraženih. Stručnjaci ističu da AY.4.2, odnosno delta-plus, još nije postao dominantan ni u jednoj od država u kojima je prijavljen.

     „Trenutno se ovaj soj nalazi u nekim zemljama, ali nigdje ne postaje dominantan. Moguće je uočiti sličnost s tzv. lambda sojem, oko kojega su ljudi u početku bili zabrinuti na lokacijama kao što su SAD ili Velika Britanija”, izjavila je dr. Roselyn Lemus-Martin sa Sveučilišta Oxford. Stručnjaci napominju da su slične mutacije viđene i u drugoj varijanti delta soja, ali to nije uvelike utjecalo na virus.

     Pitanje je li moguće da delta-plus probije zaštitu stvorenu cijepljenjem ili preboljenjem covida zasad nema preciznog odgovora. Kako je nedvojbeno utvrđeno da se ovaj soj najbrže širi među necijepljenima, zaključak za takvo što trenutno je bez logične podloge. Prema dosadašnjim britanskim iskustvima ova varijenta nema utjecaja na povećanu hospitalizaciju među oboljelima.

     Zbog svoje brze prenosivosti, najveća briga epidemiologa dosad je bila delta varijanta koronavirusa, još uvijek najdominantnija mutacija SARS-CoV-2 virusa u svijetu. Fokus znanstvenika se, međutim, snažno pomiče na delta-plus soj koji je jučer potvrđen i u Hrvatskoj. Soj je registriran u Čakovcu i nije baš veliko iznenađenje što se konačno pojavio i kod nas, pošto je u Europu stigao još prije ljeta.

    Riječ je o varijanti AY.4.2 koja je dosad otkrivena u više od 40 zemalja: od Danske, Velike Britanije, Austrije, Španjolske, pa preko niza istočnoeuropskih zemalja i Rusije do SAD-a, Izraela i Indije. Mutacije u genomu proteina šiljka daju mu veću  imunoevazivnost, odnosno mogućnost izbjegavanja čovjekova imunosnog odgovora. U kojoj mjeri je sposobniji od ranijih sojeva zaobići obrambene sustave organizma još uvijek nije dovoljno istraženo. Prema dosad provedenim kliničkim istraživanjima, delta plus djeluje nešto razornije na pluća i izaziva jače poteškoće u cijelom respiratornom traktu, te inicira probavne smetnje učestalije od ostalih sojeva.

     "Povećanje prijenosa AY.4.2 prati se od srpnja. U tijeku su epidemiološke i laboratorijske studije kako bi se procijenilo je li došlo do promjene karakteristika prijenosa ili smanjenja učinka antitijela na odbacivanje virusa“, objavili su nedavno iz Svjetske zdravstvene organizacije, dodajući da je najveći postotak zaraženih ovim virusom iz Ujedinjenog Kraljevstva. Britanska agencija za zdravstvenu sigurnost UKHSA prošlog je tjedna označila AY.4.2 kao "soj pod istragom", ali unatoč njegovom snažnom širenju još ipak nije službeno proglašen „zabrinjavajućim sojem“.

     Stručnjaci nisu posve sigurni što je dovelo do jačeg širenja ovoga soja u Britaniji i je li to posljedica mutacije ili, možda, tamošnjih minimalnih epidemioloških mjera. Trenutne britanske mjere protiv covida su vrlo slabe, praktički ih nitko i ne nadzire, pa se i ne zna u kojoj mjeri je to razlog jačem širenju ove inačice.

     Poznato je kako najrasprostranjenija delta varijanta korone dosad ima preko 50 svojih podvarijanti. Sve one imaju tipične mutacije za delta virus, ali postoje i dodatne mutacije koje ih onda svrstavaju u njegove podvarijante. Takav soj je i AY.4.2 jer ima dvije dodatne mutacije u proteinu šiljka - L452R i P871R - uz čiju se pomoć brže širi. Britansko iskustvo od početka srpnja, kada je otkriven, govori da od svih izolata obuhvaća oko 10 posto i bilježi trend porasta. Na Otoku je najviše slučajeva delte plus zabilježeno kod djece, no to medicinare puno ne iznenađuje jer je riječ o populaciji koja je najslabije procijepljena. 

     Ranije mutacije korone izazivale su u pravilu skokovite valove u broju novozaraženih. Stručnjaci ističu da AY.4.2, odnosno delta-plus, još nije postao dominantan ni u jednoj od država u kojima je prijavljen.

     „Trenutno se ovaj soj nalazi u nekim zemljama, ali nigdje ne postaje dominantan. Moguće je uočiti sličnost s tzv. lambda sojem, oko kojega su ljudi u početku bili zabrinuti na lokacijama kao što su SAD ili Velika Britanija”, izjavila je dr. Roselyn Lemus-Martin sa Sveučilišta Oxford. Stručnjaci napominju da su slične mutacije viđene i u drugoj varijanti delta soja, ali to nije uvelike utjecalo na virus.

     Pitanje je li moguće da delta-plus probije zaštitu stvorenu cijepljenjem ili preboljenjem covida zasad nema preciznog odgovora. Kako je nedvojbeno utvrđeno da se ovaj soj najbrže širi među necijepljenima, zaključak za takvo što trenutno je bez logične podloge. Prema dosadašnjim britanskim iskustvima ova varijenta nema utjecaja na povećanu hospitalizaciju među oboljelima.
