Desetero oboljelih od ATTR-CM-a, rijetke bolesti koja zahvaća srce i uzrokuje zatajenje srca, već dulje vrijeme iščekuju vijesti iz HZZO-a o dostupnosti lijeka koji za njih doslovno znači produljenje i olakšanje svakodnevnog života.
U suprotnom, mogu se samo suočiti sa spoznajom da im svaki dan može biti i zadnji, jer je prosječno preživljenje neliječenih bolesnika od postavljanja dijagnoze od dvije i pol do četiri godine, ovisno o obliku bolesti, piše živim.hr.
Transtiretinska amiloidna kardiomiopatija (ATTR-CM) nedovoljno je poznata bolest i rijetko se dijagnosticira. Nastaje zbog pogrešnog smatanja proteina transtiretina, koji se sintetizira u jetri, uzrokujući stvaranje štetnih vlakana amiloida - oni se potom talože u srčanom mišiću, što dovodi do oštećenja i zatajenja srca.
Nasljedni i divlji tip
Javlja se u dva oblika - nasljedni, odnosno genski uzrokovan, te nenasljedni, takozvani divlji tip bolesti.
Procjenjuje se da je u Hrvatskoj oko 150 oboljelih od ATTR-CM-a: desetero je dijagnosticirano i čeka na liječenje, a samo dvoje ih se trenutačno liječi odgovarajućom ciljanom terapijom.
Liječnici procjenjuju da će u naredne tri godine oko 50 oboljelih biti dijagnosticirano i kandidati za liječenje lijekom koji im produžuje život.
Riječ je o novoj ciljanoj terapiji koja je dostupna u četrnaest zemalja Europske unije, između ostalog u susjednoj Sloveniji - ali ne i u Hrvatskoj.
Primarni cilj uvođenja odgovarajuće terapije je zaustavljanje progresije bolesti, koja oboljelima onemogućuje normalno funkcioniranje jer im je život podređen čestim hospitalizacijama - i koja u konačnici ima smrtni ishod.
Zbog kontinuiranog odgađanja uvođenja spomenute terapije, zbog čega su dosad izgubljeni mnogi životi, Hrvatski savez za rijetke bolesti pokrenuo je kampanju "I moje je srce važno“.
Cilj je oboljelima od ATTR-CM-a na vrijeme osigurati dostupnim učinkovito liječenje, ali i ukazati javnosti na postojanje ove teške bolesti.
Ciljana terapija
- Savez se već neko vrijeme zalaže za dolazak ciljane terapije na listu HZZO-a. Poslali smo dopis i apel, no odgovora nema. Razni izgovori zdravstvene administracije ne smiju biti prepreka u osiguravanju odgovarajućeg liječenja i spašavanja ljudskih života.
Stoga ponovno apeliramo na sve zdravstvene institucije u Hrvatskoj da prepoznaju važnost omogućavanja terapije koja zaustavlja napredak bolesti i time produžava život oboljelima - izjavila je Sara Bajlo, dopredsjednica Hrvatskog saveza za rijetke bolesti.
Potreba za učinkovitim terapijskim pristupima je očita jer pridonosi smanjenu smrtnosti kod određenih skupina bolesnika pravovremeno liječenih za čak 50 posto.

Osim smanjenja mortaliteta, terapija, naime, zaustavlja napredak bolesti i time znatno smanjuje hospitalizaciju, za čak 32 posto, popravlja kvalitetu života oboljelima i omogućuje im svakodnevno funkcioniranje ako se primjeni na vrijeme odnosno u ranijoj fazi bolesti.
- Prognoze za amiloidnu TTR-kardiomiopatiju vrlo su nepovoljne, a jedan od najvažnijih uzroka lošeg ishoda jest kasno otkrivanje bolesti, kao i samo simptomatsko liječenje.
'Pogrešno smatanje'
Većina oboljelih od ATTR-CM-a u Hrvatskoj već se godinama liječe standardnom farmakoterapijom - ponajviše diureticima - koja je ograničena samo na olakšanje simptoma.
Odgovarajuća terapija bitno bi smanjila progresiju bolesti, smrtnost i hospitalizacije, kao i potrebe za transplantacijom srca i jetre. Ti zahvati su iznimno skupi i zahtjevni, a i rijetko se izvode jer su pogodni za vrlo mali broj oboljelih - rekla je izv. prof. dr. sc. Maja Čikeš, KBC Zagreb.
Nova generacija lijekova, objasnila je izv. prof. dr. sc. Čikeš, dizajnirana je tako da ciljano djeluju na protein transtiretin, sprječavajući njegovo slamanje odnosno "pogrešno smatanje“.
Time se smanjuju stvaranje i taloženje štetnih amiloida, usporava se proces zahvaćanja srčanog mišića, a time i napredovanje same bolesti. Oboljelima bi odgovarajuće liječenje novim učinkovitim lijekom omogućilo da kvalitetno žive s tom rijetkom bolešću, kao i oni koji pate od ostalih kroničnih bolesti.
Nenasljedni, takozvani divlji oblik bolesti, češći je i uglavnom pogađa muškarce u dobi iznad 60 godina. Nasljedni oblik, pak, podjednako pogađa oba spola te se obično dijagnosticira nešto ranije, u petom desetljeću života, iako se neki simptomi oštećenja srca i živčanog sustava mogu javiti i desetljećima ranije.

Za Hrvatsku je specifično to što je većina dijagnosticiranih bolesnika s nasljednim tipom porijeklom iz istog kraja - Imotskog i okolice - što Dalmatinsku zagoru čini endemskim područjem za ovu bolest, piše živim.hr.
Najčešći simptomi
Život s ovom rijetkom, progresivnom i za život opasnom bolešću karakterizira bol ili ukočenost u donjem dijelu leđa i nogama, nepravilan rad srca, kratkoća daha ili utrnutost šaka i ruku te gastrointestinalni problemi kao što su proljev, zatvor, mučnina, osjećaj rane sitosti.
Mogu se javiti i simptomi kao što su omaglica ili nesvjestica prilikom ustajanja iz ležećeg ili sjedećeg položaja, nemogućnost zadržavanja mokraće ili spontano puknuće tetive mišića bicepsa na rukama. Ako se pravodobno ne liječe, ti simptomi dovode do zahvaćenosti živčanog sustava i zatajivanja srca.

Svaki život je važan i gubitak ne samo kvalitete nego i života vrlo brzo nakon dijagnoze može se spriječiti dobivanjem pravovremene, odgovarajuće terapije.
A životi oboljelih od ATTR-CM-a sada su u rukama odgovornih osoba u hrvatskom javnom zdravstvu - oni im mogu omogućiti produljenje života, poručili su iz Hrvatskog saveza za rijetke bolesti.

Desetero oboljelih od ATTR-CM-a, rijetke bolesti koja zahvaća srce i uzrokuje zatajenje srca, već dulje vrijeme iščekuju vijesti iz HZZO-a o dostupnosti lijeka koji za njih doslovno znači produljenje i olakšanje svakodnevnog života.
U suprotnom, mogu se samo suočiti sa spoznajom da im svaki dan može biti i zadnji, jer je prosječno preživljenje neliječenih bolesnika od postavljanja dijagnoze od dvije i pol do četiri godine, ovisno o obliku bolesti, piše živim.hr.
Transtiretinska amiloidna kardiomiopatija (ATTR-CM) nedovoljno je poznata bolest i rijetko se dijagnosticira. Nastaje zbog pogrešnog smatanja proteina transtiretina, koji se sintetizira u jetri, uzrokujući stvaranje štetnih vlakana amiloida - oni se potom talože u srčanom mišiću, što dovodi do oštećenja i zatajenja srca.
Nasljedni i divlji tip
Javlja se u dva oblika - nasljedni, odnosno genski uzrokovan, te nenasljedni, takozvani divlji tip bolesti.
Procjenjuje se da je u Hrvatskoj oko 150 oboljelih od ATTR-CM-a: desetero je dijagnosticirano i čeka na liječenje, a samo dvoje ih se trenutačno liječi odgovarajućom ciljanom terapijom.
Liječnici procjenjuju da će u naredne tri godine oko 50 oboljelih biti dijagnosticirano i kandidati za liječenje lijekom koji im produžuje život.
Riječ je o novoj ciljanoj terapiji koja je dostupna u četrnaest zemalja Europske unije, između ostalog u susjednoj Sloveniji - ali ne i u Hrvatskoj.
Primarni cilj uvođenja odgovarajuće terapije je zaustavljanje progresije bolesti, koja oboljelima onemogućuje normalno funkcioniranje jer im je život podređen čestim hospitalizacijama - i koja u konačnici ima smrtni ishod.
Zbog kontinuiranog odgađanja uvođenja spomenute terapije, zbog čega su dosad izgubljeni mnogi životi, Hrvatski savez za rijetke bolesti pokrenuo je kampanju "I moje je srce važno“.
Cilj je oboljelima od ATTR-CM-a na vrijeme osigurati dostupnim učinkovito liječenje, ali i ukazati javnosti na postojanje ove teške bolesti.
Ciljana terapija
- Savez se već neko vrijeme zalaže za dolazak ciljane terapije na listu HZZO-a. Poslali smo dopis i apel, no odgovora nema. Razni izgovori zdravstvene administracije ne smiju biti prepreka u osiguravanju odgovarajućeg liječenja i spašavanja ljudskih života.
Stoga ponovno apeliramo na sve zdravstvene institucije u Hrvatskoj da prepoznaju važnost omogućavanja terapije koja zaustavlja napredak bolesti i time produžava život oboljelima - izjavila je Sara Bajlo, dopredsjednica Hrvatskog saveza za rijetke bolesti.
Potreba za učinkovitim terapijskim pristupima je očita jer pridonosi smanjenu smrtnosti kod određenih skupina bolesnika pravovremeno liječenih za čak 50 posto.

Osim smanjenja mortaliteta, terapija, naime, zaustavlja napredak bolesti i time znatno smanjuje hospitalizaciju, za čak 32 posto, popravlja kvalitetu života oboljelima i omogućuje im svakodnevno funkcioniranje ako se primjeni na vrijeme odnosno u ranijoj fazi bolesti.
- Prognoze za amiloidnu TTR-kardiomiopatiju vrlo su nepovoljne, a jedan od najvažnijih uzroka lošeg ishoda jest kasno otkrivanje bolesti, kao i samo simptomatsko liječenje.
'Pogrešno smatanje'
Većina oboljelih od ATTR-CM-a u Hrvatskoj već se godinama liječe standardnom farmakoterapijom - ponajviše diureticima - koja je ograničena samo na olakšanje simptoma.
Odgovarajuća terapija bitno bi smanjila progresiju bolesti, smrtnost i hospitalizacije, kao i potrebe za transplantacijom srca i jetre. Ti zahvati su iznimno skupi i zahtjevni, a i rijetko se izvode jer su pogodni za vrlo mali broj oboljelih - rekla je izv. prof. dr. sc. Maja Čikeš, KBC Zagreb.
Nova generacija lijekova, objasnila je izv. prof. dr. sc. Čikeš, dizajnirana je tako da ciljano djeluju na protein transtiretin, sprječavajući njegovo slamanje odnosno "pogrešno smatanje“.
Time se smanjuju stvaranje i taloženje štetnih amiloida, usporava se proces zahvaćanja srčanog mišića, a time i napredovanje same bolesti. Oboljelima bi odgovarajuće liječenje novim učinkovitim lijekom omogućilo da kvalitetno žive s tom rijetkom bolešću, kao i oni koji pate od ostalih kroničnih bolesti.
Nenasljedni, takozvani divlji oblik bolesti, češći je i uglavnom pogađa muškarce u dobi iznad 60 godina. Nasljedni oblik, pak, podjednako pogađa oba spola te se obično dijagnosticira nešto ranije, u petom desetljeću života, iako se neki simptomi oštećenja srca i živčanog sustava mogu javiti i desetljećima ranije.

Za Hrvatsku je specifično to što je većina dijagnosticiranih bolesnika s nasljednim tipom porijeklom iz istog kraja - Imotskog i okolice - što Dalmatinsku zagoru čini endemskim područjem za ovu bolest, piše živim.hr.
Najčešći simptomi
Život s ovom rijetkom, progresivnom i za život opasnom bolešću karakterizira bol ili ukočenost u donjem dijelu leđa i nogama, nepravilan rad srca, kratkoća daha ili utrnutost šaka i ruku te gastrointestinalni problemi kao što su proljev, zatvor, mučnina, osjećaj rane sitosti.
Mogu se javiti i simptomi kao što su omaglica ili nesvjestica prilikom ustajanja iz ležećeg ili sjedećeg položaja, nemogućnost zadržavanja mokraće ili spontano puknuće tetive mišića bicepsa na rukama. Ako se pravodobno ne liječe, ti simptomi dovode do zahvaćenosti živčanog sustava i zatajivanja srca.
