Karcinom gušterače spada među rjeđe oblike raka probavnog sustava. Nažalost, obično se kasno otkriju smetnje i više od 95 posto pacijenata umire u roku od dvije godine nakon dijagnoze, prenosi Živim pisanja Onkologije..

Što je karcinom gušterače?

Velika studija pokazala je da kombinacija pušenja, dugotrajnog dijabetesa i loše prehrane povećava rizik od raka gušterače iznad rizika bilo kojeg od ovih čimbenika pojedinačno.

Zašto ga je teško uočiti?

Dr. Tang navodi da je ovaj tip karcinoma teško otkriti jer je gušterača mali organ koji se nalazi duboko u abdomenu i skriven je iza drugih organa. Karcinom gušterače spada među rjeđe oblike raka probavnog sustava. Nažalost, obično se kasno otkriju smetnje i više od 95 posto pacijenata umire u roku od dvije godine nakon dijagnoze, prenosi Živim pisanja Onkologije..

U Hrvatskoj se godišnje dijagnosticira više od 600 novih slučajeva. To je bolest koja zahvaća veliku žlijezdu koja je dio probavnog sustava, smještenu iza želuca i ispod jetre.

Što je karcinom gušterače?

Gušterača je organ čije su glavne funkcije oslobađanje probavnih enzima u crijeva za razgradnju hrane te lučenje hormona inzulina i glukagona za reguliranje razine šećera u krvi.

- Rak gušterače se događa kad normalne stanice mutiraju u abnormalne i rastu bez kontrole - rekao je dr.med. Afshin Safa, onkolog, za Eat This, Not That!.

Karcinom se obično javlja u tkivu gušterače, a može se pojaviti nekoliko vrsta izraslina, uključujući kancerogene i nekancerogene. Najčešći tip raka koji nastaje počinje u stanicama koje oblažu kanale koji izvode probavne enzime iz gušterače, a naziva se duktalni adenokarcinom gušterače, stoji na stranicama Klinike Mayo.

Certificirani hematolog/onkolog iz Los Angeles, dr. med. Ronald Tang navodi da rak gušterače zahvaća 13 na 100 000 ljudi. Ima jednu od najviših stopa morbiditeta od svih vrsta raka zbog činjenice da kada se dijagnosticira, obično je uznapredovao ili metastazirao.

Znakovi koji ukazuju na ovaj tip karcinoma

Klinika Mayo navodi da je bol simptom na kojeg trebate obratiti posebnu pozornost. Naime, to je prvi simptom na kojeg se žale brojni oboljeli.

- Rastući tumor pritišće živce u vašem abdomenu te tako uzrokuje bol koja može postati jaka. Lijekovi protiv bolova mogu vam pomoći. Tretmani, poput zračenja i kemoterapije, mogu usporiti rast tumora i donekle ublažiti bol - navode iz Klinike Mayo.

Ovo su još neki simptomi na koje trebate obratiti pažnju:

probavni problemi - gubitak apetita, probavne smetnje i mučnina česti su kod osoba s rakom gušterače. Ovi simptomi se često događaju kada bolest utječe na probavu hrane i apsorpciju nutritivnih tvari. Mnogi bolesnici s rakom gušterače imaju proljev, zatvor ili oboje

bol u želucu - bol u trbuhu ili u sredini leđa može biti uzrokovana tumorom. Zbog položaja, tumor može pritiskati živac, organe u blizini, pa i probavni trakt

gubitak tjelesne težine - gubitak težine može biti uzrokovan nepotpunom probavom zbog raka ili samog raka (tumorska kaheksija), koja značajno utječe na proces liječenja i stopu preživljenja. Koža i oči žučćkaste su boje - razlog "žutilu" je taj da se rak gušterače obično počne razvijati na njegovoj "glavi", koja je smještena u blizini žučnog kanala. Tumor vrši pritisak na žučni kanal, a on zbog toga ne uspijeva prenijeti žuč iz jetre i žučne vrećice kroz gušteraču do tankog crijeva.

Sve vas svrbi - prekomjerni svrab po cijelom tijelu rani je znak koji tijelo šalje u slučaju žutice i raka gušterače. Kako se bilirubin nakuplja u krvi, pa i u koži, uzrokuje svrab i prije nego koža postane žuta.

Što povećava rizik?

- Čimbenici rizika uključuju zlouporabu duhana i alkohola kod pacijenata koji pate od kronične upale gušterače uzrokovane alkoholom ili žučnim kamencima - kaže dr. Safa.

Svakako je važno naglasiti da svijest o čimbenicima rizika (genetska predispozicija, dob, pušenje, dijabetes) može dovesti do ranije i bolje procjene raka gušterače kod pacijenata koji imaju simptome zbog koji se sumnja na navedenu zloćudnu bolest.

Klinika Mayo navodi da ovi čimbenici mogu povećati rizik:

Dijabetes

Kronična upala gušterače (pankreatitis)

Obiteljska povijest genetskih sindroma koji mogu povećati rizik od raka, uključujući mutaciju gena BRCA2, Lynchov sindrom i sindrom obiteljskog atipičnog malignog madeža (FAMMM)

Starija dob, budući da se većini ljudi dijagnoza postavlja nakon 65. godine

Velika studija pokazala je da kombinacija pušenja, dugotrajnog dijabetesa i loše prehrane povećava rizik od raka gušterače iznad rizika bilo kojeg od ovih čimbenika pojedinačno.

Zašto ga je teško uočiti?

Dr. Tang navodi da je ovaj tip karcinoma teško otkriti jer je gušterača mali organ koji se nalazi duboko u abdomenu i skriven je iza drugih organa. Nažalost, do trenutka kada se jave simptomi kao što je nelagoda u abdomenu, rak je obično uznapredovao ili metastazirao. Ne postoje jasne smjernice za probir raka gušterače za razliku od raka dojke, debelog crijeva i prostate.

Međutim, ako vi ili vaš liječnik sumnjate na ovaj tip karcinoma, može se napraviti nekoliko testova:

Slikovni testovi - Ultrazvuk, CT skeniranje, MRI skeniranje ili test koji se zove ERCP (endoskopska retrogradna kolangiopankreatografija).

Biopsija

Kako se liječi?

Liječenje uključuje operaciju, zračenje i/ili sustavnu kemoterapija. Djelomično ili potpuno uklanjanje gušterače operacijom jedino je što potencijalno može dovesti do izlječenja. Kemoterapija i radioterapija mogu poboljšati preživljenje, objavio je Živim.
